Patient Information

NEW PATIENTS 

When you come for your initial appointment, please arrive 15 minutes early to complete any necessary paperwork. New patients should bring the following:

  • Patient Information Forms
  • Medical History
  • Insurance Card

Please select the physician with whom you have scheduled your appointment on the Patient Forms Page to download the appropriate forms.

  • Insurance card
  • Medical records (if not already forwarded to our office)
  • Any test results, X-rays, Films, Disks, MRI and CT Scans
  • A list of all current medications (prescription and over-the-counter)
  • All medical insurance cards


FOLLOW-UP APPOINTMENTS 

Please arrive 10 minutes prior to your scheduled appointment.

REFERRALS/INSURANCE PRE-APPROVAL

Some insurance companies require a referral for an appointment from your primary care provider. If you have any questions, please contact your primary care physician or call our office for confirmation. You may be seen only for what is recommended by your doctor and/or the referring agency.

CANCELLATIONS 

Please give our office a 24-hour notice if you are unable to keep your appointment for any reason.

OFFICE HOURS 

Our office is open Monday through Friday, 8 am until 5 pm.

INSURANCE 

We accept all major insurance plans except TennCare. We also participate in two Medicare Advantage plans: Cariten Sr. and Windsor. Our insurance specialists will help you navigate your insurance coverage, file claims and help with pre-authorization for treatment. Patients are required to pay insurance deductibles or co-payments at the time of service. In addition to cash and checks, we accept Visa, Mastercard, Discover and American Express.

Please call our office if you have any questions about insurance or to make payment arrangements. We are happy to help.

PRESCRIPTIONS 

If you need a prescription refilled before you next appointment, please call our office to make arrangements. When calling, be sure to have ready all prescription information and the phone number of your pharmacy. Not all prescriptions can be filled the same day, so please allow at least 48 hours for a prescription refill.

MEDICAL RECORDS 

Medical records can be transferred to another physician or medical practice with a proper, written request from the patient.

EMERGENCIES 

In the event of an emergency, one of our physicians is on call 24 hours a day, seven days a week. Please call and one of our physicians will contact you as soon as possible. If you are experiencing a life-threatening situation, please call 911 immediately.

HOSPITALS 

Our physicians have privileges at Park West and Baptist West Hospitals in Knoxville, Tennessee.
